TIME IS RUNNING OUT -- ALL RESIDENTS MUST CONNECT BY NOVEMBER, 2008!

BEFORE WE CAN ISSUE A SEWER CONNECTION PERMIT:

1.  Apply for a septic abandonment permit from McHenry County Health Department.
BRING THIS PERMIT WITH YOU!
As of 7/13/07 the fee for the county permit is $100.00 (subject to change)

2.  Pick your plumbing contractor--
--We do not recommend any one plumber over another, but we do have some phone numbers if you need help finding one.  Each contractor's price varies; there are several factors that affect pricing for each individual property.  Please do not ask us for prices -- we don't know!

a.) We must have a copy of their valid plumber's license,
b.) a copy of their insurance bond listing the Village of McCullom Lake as additionally insured.

3.  Bring all of these items to the Village office during regular office hours with a check for $250.00 written to the Village of McCullom Lake. 

You will fill out the permit form, we will make copies of all the info we need, and give you the original permit. Your contractor needs to call the village office at least 24 hours prior to needing the inspection. Your contractor and our plumbing inspector will sign off on the permit, give you a copy, and return the other copies to the village office for processing.

**Please keep in mind, your permit fee covers one inspection.  If your plumber is not ready when our inspector arrives at the scheduled time, or has done something incorrectly that results in additional inspections being required, you will be charged $25.00 for each additional inspection.
